Therapeutic algorithm for treatment of acute appendicitis. Conservative treatment with antibiotics is the first choice, except in cases of acute pan-peritonitis or severe inflammation with extensive abscess formation at the time of initial diagnosis. Early surgery may also be performed if it is determined that antibiotic treatment is not curative. IA is performed after antibiotic treatment in cases with fecal stones, appendix diameter ≥10 mm, and recurrent cases
